Features & Capabilities

New in 4.0: a calmer design with the result first, a dose colour guide, route conversion, and switching timing free for everyone.

Opioid Conversion Calculator is built for health professionals who need a clear, efficient way to compare opioid doses and review the clinical context behind a conversion. Built by a pharmacist.

CALCULATE AND COMPARE

  • Calculate total oral morphine equivalent (OME) across multiple opioids and routes: oral, parenteral, rectal, sublingual and transdermal.
  • See every converted dose at once, after applying a chosen Target OME percentage for incomplete cross-tolerance.
  • View conservative, rounded-down target and breakthrough dose suggestions.
  • See where a daily dose sits: the header warms from orange at 50 mg/day (CDC 2022) to red at 120 mg/day (Opioids Aware). Adjust the thresholds or turn the guide off.
  • Methadone uses its own dose-dependent model, and its results are clearly marked as not a starting dose.
  • Convert the same opioid between routes, such as oral and parenteral morphine, with the method and sources shown.

EVERY RESULT, IN CONTEXT

  • Each result opens with the converted dose, then products, clinical information, breakthrough dosing and switching timing.
  • Read practice points alongside formulation-specific pharmacology, with links to the supporting sources.
  • Check when to start the new opioid when switching between patches, modified-release opioids and CSCI. Switching timing is also in the menu, free for everyone.
  • View official products and strengths for Australia, the United States, Germany, France and Spain, chosen independently from the app language.

TAILOR THE WORKFLOW

  • Edit conversion factors and choose which opioids appear, with Pro.
  • Export and import settings when moving between devices, with Pro.
  • Works offline, including on first launch.
  • Use the app in English, French, German, Italian, Portuguese (Brazil) or Spanish.

PRO ACCESS

The calculator stays free: the combined OME and every converted dose need no account or purchase. Pro adds breakthrough dosing, switching timing, products and clinical information to every result, plus custom conversion factors, opioid visibility and settings transfer. Your first detailed results are free to try. Available plans, prices and any introductory offer are shown using the information supplied by the App Store before purchase. Purchases can be restored in the app.

CLINICAL USE

Equianalgesic conversions are estimates and do not replace clinical judgement. Consider incomplete cross-tolerance, patient factors, formulation, route, local guidance and monitoring when interpreting a result. Review the references and terms of use before using the calculator in clinical practice.

I downloaded the app as a alternative to the old CDC guideline app, which is no longer available for download. I ran it through a couple of calculations. It calculated that hydromorphone 4 mg = 20 MME. This is incorrect as the answer should have been 16 MME. Also, oxymorphone is missing from the conversion list. It is still a medication and it does still exist. There should be a tool for calculating its conversion. Very poorly researched, very misleading.
