Before and after each clinic or hospital visit, a licensed Truentity clinical team member performs a virtual medication reconciliation service on behalf of the provider to assess the patient’s current medication therapies and generate an accurate medication history. As a result, this advanced platform improves healthcare outcomes for patients and enables providers to improve efficiency and reduce costs while increasing their STAR ratings and HCAHPS scores.
