Polyphonic™ Care Pro

Polyphonic™ Care Pro is a digital care coordination communication tool that allows healthcare professionals to connect, educate and support their patients through an entire care pathway.


Healthcare Practitioners with an approved account will be able to use Polyphonic™ Care Pro to:

• View their active patient cohort
• Track individual patients' progress through their treatment pathway;
• View which materials and checklists patients have completed Please be aware the app has a limited subset of features compared to the portal. The portal https://eu.polyphonic.jnjmedtech.com/carepro allows administration and analytics.

• Polyphonic™ Care Pro does not qualify as a medical device per applicable regulations, including the EU Medical Device Regulation No 2017/745.
• Polyphonic™ Care Pro does not perform an action on data different from storage, archival, communication or simple search.
• Polyphonic™ Care Pro is not intended for patient diagnosis or treatment.  Healthcare professionals only are responsible for taking decisions related to patient diagnosis or treatment.
• If patients have any questions related to their state of health at any point in their treatment, they should contact their healthcare provider directly.
