HealthJay is a care engagement app that uses a Building-Block like interface offering person-centered access to healthcare recipients. Clinical care, chronic care, and social determinants of health are all delivered including walkie-talkie video chat with care providers. Users can manage care events in this ultra simple-to-use app using a whole-health approach.
