The American Diabetes Association® is excited to bring you Connect the Dots—a FREE program that helps you understand the impact of life’s ups and downs on your glucose (blood glucose or interstitial glucose) levels, putting you in control of your diabetes management with customized coaching to help you thrive.

A Program as Unique as You Your body and journey living with diabetes is unique, and there is no one-size-fits-all approach. You choose when and how to get information; whether that be through our coaches, website or phone app powered by Good Measures—making it convenient to manage your diabetes.

HOW IT WORKS:

1. Meet your Coach – A diabetes educator will use a customized approach to empower you to take control of your diabetes.
2. Monitor your Blood Sugar – Use your continuous glucose monitor (CGM) or blood glucose monitor (BGM) to know your numbers.
3. Connect the Dots – Put the pieces together for breakthrough moments throughout your diabetes journey.

HOW THIS PROGRAM CAN HELP

• Use technology to know your numbers.
• Understand how to reduce your risk of long-term complications.
• Learn how to create your plate to support healthy eating.
• Understand where a small food swap can support healthful eating patterns.
• Understand the role of medication and how to approach your health care provider.
• Create an action plan to increase and maintain physical activity.

Connect the Dots integrates with the Health app to support automatic import of steps and calories burned from workouts to the Connect the Dots exercise log and body weight measurements to the Connect the Dots weight tracking.
