DBm-Health

DBm-Health™ is intended for patients with, or at risk of diabetes who have been advised by their health care professionals to download this app.

Features and benefits:

• Upload blood glucose readings either directly from your personal glucometer via Bluetooth, or by entering them manually
• Track your readings and view historical readings as a list, in diary format, or as a graph
• Easily filter your readings to show trends
• Readings are coloured red, green, or blue to help you keep on track with your blood glucose monitoring
• Add comments, medication taken, and meal types to a reading
• Request a call back from your care team
• Receive care guidance from your care team direct to your app
• Read helpful information about how to self-manage your blood glucose levels including information about food and diet, weight management, activity and fitness
• All personal data is encrypted and sent securely to your NHS trust using HTTPS.

Your care team will need to issue you with an activation code for you to access all the features in DBm-Health.
