The DEARhealth app helps patients with a chronic disease manage their health, in collaboration with their healthcare providers. Following the principles of Value-Based Healthcare, we focus on what matters to you as a patient. The care you get should bring you and your health value.

This is why the DEARhealth app measures the value of your care with a disease-specific Health Assessment. This focuses on three aspects of your overall health: disease control, quality of life and daily productivity. You also get insight into the care pathway set out for you by your healthcare provider. This means you’ll know which questionnaires are planned when for the upcoming months. Continuous monitoring of your Health Assessment allows you and your healthcare provider to react to any changes, e.g. update your care pathway.

DEARhealth allows you to: • Fill in your Health Assessment and check your scores over time • View your individualized care pathway with upcoming questionnaires • Learn about your disease with the MyAcademy program • Access additional Wellness programs such as MyMenu and MyCoach

To use the DEARhealth app, you must create an account with your healthcare provider. Visit www.dearhealth.com for a listing of supported providers. If your provider is not listed or you need help setting up the account, please contact them directly.
