Cardio Care

Cardio Care is an application designed to improve the care of patients with heart failure, as well as facilitate the link with their treating physicians.

It was created based on the recommendations addressed to patients for self-monitoring at home and surveillance of alarm data, through the daily measurement of different parameters such as vital signs, anthropometry, habit of fluid consumption and physical activity, as well as their perception of well-being. .

With a simple and straightforward format, the application allows patients to record their daily measurements, which are sent to the application's medical portal so that their treating physician can monitor the data sent by the patient in real time. The data is presented to the doctor in the form of tables and graphs to see the trends in the behavior of the different parameters of the patient day by day and that allow timely detection of any decompensation of the patient's heart failure condition that determine a visit to the doctor in person (office or hospital).

This application allows the patient to be in close contact with their doctor through a system of comments and alerts that the patient can send from the application to the medical portal.

In addition, it has the purpose of providing medical education to the patient and/or their caregiver, as well as promoting adherence to their pharmacological treatment through instructions and reminders within the application with self-care measures and identification of alarm data to request care. as well as medication intake reminders.
