Referrals are made easy with face sheet photo capture and secure mobile form submission, ensuring quick and seamless hospice transitions.

Users can locate VITAS hospice offices and inpatient units by zip code or state, and connect directly with admissions staff via call or email. Additionally, the app provides access to educational webinars on end-of-life care, some offering CE/CME credits. Shared resources and information enable effective communication with patients, families, and colleagues.
