Vodafone Home Health is an application that allows healthcare professionals to remotely maintain a broad view of the patient and anticipate, whenever possible, an episode causing hospitalization.
Through the application, the patients are able to make measurements, interacting with the application and the medical equipment, and the measurement data is sent in real time to the medical platforms where health officials are able to check the patient's health status.
