This is a tool used to evaluate cooperative health. It is used to record the survey of the cooperative so that to know its health to the coffe.it is has many surveys for different cooperatives that have category questions. It has two parts: insert survey part and edit survey. The surveyor has to insert his name in the cooperative and also insert the answers for questions for the survey. If user want to edit he can edit that day only after day you can’t edit.
