Lister Health Metrics (LHM)

Lister Health Metrics (LHM) is a companion app for patients enrolled in Lister Healthcare’s Remote Patient Monitoring (LHM) program. It is used with your care team’s direction—not as a standalone health product.

IMPORTANT — NOT A MEDICAL DEVICE
LHM is not a medical device. It does not diagnose, treat, cure, or prevent any disease or medical condition. It does not provide emergency services. If you have a medical emergency, call your local emergency number immediately. Always follow your healthcare provider’s instructions.

HOW THE APP WORKS
• For enrolled patients only (account provided by your healthcare organization).
• Displays vitals and care information shared with your clinical team (e.g., blood pressure, pulse/heart rate, weight, glucose—based on your care plan).
• Supports secure messaging and appointment information with your care team.
• Vitals may be entered manually in the app where your program allows, and/or received from clinician-assigned monitoring devices when supplied by your provider.

HARDWARE / DEVICE REQUIREMENTS
This app does not replace clinical monitoring devices. Depending on your care plan, your provider may assign compatible LHM devices (such as a blood pressure monitor, pulse oximeter, weight scale, and/or glucose meter). Device types vary by patient. The app cannot provide full LHM services without enrollment in the Lister Healthcare program and, when applicable, use of provider-approved devices or approved manual entry as directed by your care team.
