Your healthcare team uses this app to share important information with you and your loved ones based on your specific health needs – such as educational materials (videos and PDFs), health and wellness resources, mindfulness and relaxation tools, surveys, contact information and more.

Organizations provide this app to help patients to learn about recovery, self-care and wellness. The delivery of information and resources via a mobile app extends opportunities for individuals to participate in their own healthcare journeys. Most organizations provide this app to either supplement or replace paper-based and television-based delivery of patient education and resources.
