BridgIT - Wellbridge Health

Wellbridge Health offers an innovative, interactive chronic disease management program designed to improve quality of life, reduce avoidable emergency room visits and hospitalizations, improve self-management of chronic disease(s), and lower healthcare spend.

This program can also be used for behavioral health -- anxiety and depression -- in combination with chronic disease management or stand-alone.

The BridgIT - Wellbridge Health app combines simple 7-day/week remote symptom monitoring through our customizable smartphone app and weekly face-to-face HIPAA-compliant video chat telehealth coaching. All of our members are partnered with a dedicated master’s trained health coach.
