User can add their plan notes along with date. User will have to add date & time of appointment along with doctor’s name. so on day of appointment user receives notification for it. User can also mark their follow-up for the same.

Users can view their history within the app and any notes they have provided.

Very interactive and helpful app, at first I was skeptical to see if this app would really do what was advertised, happy to say it did not disappoint.
