Folia: Chronic Symptom Tracker helps people with chronic and rare conditions, caregivers, and those seeking a diagnosis record symptoms, medications, flare-ups, mental health, pain, sleep, nutrition, triggers, and treatments in a personalized journal. Entries can use scales, counts, yes/no responses, or custom tags, with support for scheduled and as-needed medications and caregiver tracking.

Turn daily health data into clear graphs, reports, and insights to identify patterns, understand potential medication side effects, and prepare for medical appointments. Through the optional Data Dividends program, eligible users may contribute de-identified health data to research and receive quarterly rewards, such as gift cards. Folia is free to use, offers privacy controls, and lets users export or delete their data.
