Health and Wellbeing Network is a tool created to improve home care for the elderly or those in a situation of dependency. The app facilitates coordination between health professionals, social services, caregivers and territorial entities, to guarantee a more agile, humane and effective response to the needs of the people served.
With this application, we want to avoid situations of isolation, unnecessary hospitalizations or institutionalizations that could be prevented with better communication. The system favors early detection of risk situations and continuous monitoring, always focusing on the person and respecting their autonomy and wishes.
