MOMHD-DH

MOM-HD is a healthcare app for pregnant women with hypertensive disorders of pregnancy (HDP) and/or gestational diabetes (GDM).
Developed in collaboration with Rutgers University, USA, the app helps patients self-manage HDP and/or GDM by increasing their knowledge and self-efficacy to monitor and control their blood pressure and/or blood glucose during pregnancy.
Patients can record their daily blood pressure and/or blood glucose readings at home. The app provides instant feedback on whether each reading falls within or exceeds clinically recommended ranges, and alerts the care team when values are outside the safe range. It also generates easy-to-read visual summaries of blood pressure and/or glucose trends over time, helping patients stay aware of their health and make informed diet and lifestyle choices.
Readings are shared with a provider-facing web portal, where healthcare providers can review patient data, track progress, and guide treatment and counseling. This supports clinical decision-making and timely interventions for maternal and fetal well-being.
The app also includes customizable reminders for blood pressure and/or glucose testing and multimedia video- and text-based educational modules.
