2023-04-10	Si*****	App Constantly Crashes! I got a pop up on my phone (the first time I opened it)asking if I'd like to clear the cache for this app because it "has a bug". It would be nice to have ANY copy of my card. Most medical places would like to scan the copy but since Moda is "going green", we are forced to do it this way and the app doesn't even work. Way to go, Moda😒🙄

2021-09-14	An*****	Crashes on Samsung z flip 3. App opens normally, shows login info but when I hit log in it crashes. When I choose leave feedback it says data is not connected but it is. Worked grea on .my Samsung s10e. Hopefully an update can be created to fix this problem as it is a very handy little app.

2021-01-05	De*****	Functionally, this fine, so why 2 stars? I went through an abusive relationship and bad divorce. Since my ex was covered, her card is still there, even though we've been divorced for more than a year. I've been purging her wherever possible, so I'm not adding this small bit of her back into my life.
