myCareHalo

myCareHalo is a medical software app used for remote patient monitoring. Membership requires enrollment into the program through your health plan or health care provider. Please contact your health care provider to request an account. This application supports reading collected data from myCareHalo provided connected Bluetooth devices.

Member devices will be determined by your health care provider. Device options include blood pressure, blood glucose, thermometer, pulse oximeter, and body analyzer scale. The vital information collected in the app will be made available to your Healthcare Provider. You can see your collected vital information in the History section of the application.

The myCareHalo application is not for emergency use.
