This app allows you to complete necessary paperwork and screenings before your appointment to ensure you’re able to focus on what really matters during your time with your team.


To get started, you can create your account when you are invited to an appointment by a provider. You can access the link in your email which will then prompt you to finish setting up your account and download the app.
