Coming Home helps you stay connected to your care experience between scheduled moments of support.

The app is available to clients whose care provider or organization uses Coming Home. Once your provider gives you access, you can:

View your assignments
See goals, tasks, and challenges assigned by your provider and track progress from your phone.

Message your provider
Stay connected with your assigned care provider or support team through secure in-app messaging.

Reflect privately
Use the journal to write down thoughts, experiences, or progress in a private space.



Coming Home is designed to extend support between moments of care. It does not replace your provider, professional care, crisis services, or emergency support.

Provider messaging may not be monitored in real time. If you are in immediate danger or need urgent help, call 911 or your local emergency number.

Access to the app is provided through participating organizations and care providers.
