The AdaptiveTelehealth Patient Portal gives you secure, convenient access to your care team and your health information from your mobile device. Sign in to manage your care, communicate with your providers, and stay on top of your appointments — all in one place.

This app is intended for established patients of practices that use the AdaptiveTelehealth platform. You will need login credentials provided by your healthcare provider to use the app.

What you can do in the app
• Consent for Treatment — Review and electronically sign consent forms required for your care.
• Forms — Complete and submit intake and clinical forms requested by your provider.
• Secure Messaging — Communicate directly and privately with your care team.
• Scheduling — Request, view, and manage your upcoming appointments.
• Video Conferencing — Join secure telehealth video visits with your provider.
• Document Upload & Sharing — Upload documents and images and share them securely with your care team.

Privacy and security you can trust
Your privacy is our priority. The AdaptiveTelehealth platform is independently certified to the ISO/IEC 27001 information security standard by a third-party auditor. Protected Health Information (PHI) is transmitted and stored securely.

Right to Delete
You remain in control of your data. You can request deletion of your information at any time using the Contact form within the app or via our public Contact page at https://www.adaptivetelehealth.com/contact/. Our team will process your request in accordance with applicable laws and regulations.
