TailorPath is a HIPAA-compliant case management platform designed to streamline hospital workflows and enable personalized care across three user roles: Hospital Admins (web), Doctors (web), and Patients (mobile). The system supports secure onboarding and management of doctors and patients, role-based dashboards with operational and clinical insights, and structured tools for delivering individualized care.

Doctors can assign and manage questionnaires and intervention plans, including Goal Attainment Scaling (GAS) for measurable progress tracking. The platform includes LLM-assisted goal and GAS generation (without sending identifiable patient data), secure 1:1 messaging between doctors and patients (with optional file sharing), and features such as reminders, scheduling, and confidential clinician notes (hidden from patients). Patients use a dedicated mobile app to complete onboarding, fill assigned questionnaires using accessible modalities (touch/voice/communication board), view assigned plans, and submit self-monitoring updates according to the clinician-defined frequency.

TailorPath is built as a secure cloud-based standalone system (with future interoperability options), emphasizing encryption in transit and at rest, audit logging, and strict role-based access control. The solution is designed to improve care coordination, patient engagement, and outcome tracking—especially for populations that benefit from structured, measurable, and accessible support tools.
