The application brings information about the health card number and the patient's personal data provided through the registration of the Municipal Health Secretariat (SMS). During a visit, the health agent will perform a pre-consultation requesting information related to the patient's health. The data is sent to the Ministry of Health and the Department of Health. The saved information is kept in the application and can be accessed by health professionals whenever necessary.
