No more hassle in appointments and documentations, everything is done in one place where I am able to record, edit and export in either pdf or excel files. The free version has most of the options for me to specify for my own use in various form and so easy to create. Thanks Jotform !

Center for Ministry Studies '		 en I've been using JotForm for about 4 years and, aside from a couple of issues, LOVE it! I especially like the fallibilities in the following areas: ease of building a form (once past the learning curve which isn't that difficult!), email confirmations to submitters, email back to us with a PDF attachment, ability to download responses in multiple ways. I really could go on and on. There are two features I'd like to see: 1) a standard PDF format of responses that is simple and compact and 2) a 'disabled form'page that is attractive and more editable to a user needs, currently that page looks terrible.
