- Click on the "Edit autofill inputs" button on the top of page in left corner.
- Fill in the information, reading the notes below the fields.
- Fill in the State & District. Ensure spellings are correct and words are with First Letter CAPS.
- Save the inputs by clicking on "Save" button.
