## Description

The Heart Failure app has been fully endorsed by C&amp;M Cardiac Board and is an integrated end-to-end pathway, from primary care through to tertiary care, supporting improved access and outcomes for the management of patients with Heart Failure.<br><br>The approach to pathway development has been multidisciplinary involving; patients, GPs, Cardiologists, rehab teams, Heart Failure specialist nurses, pharmacists and other healthcare specialists to support integration across organisational boundaries.<br><br>The application allows access to the Heart Failure pathway to a national audience with the aim of improving prescribing practices in primary, secondary and tertiary care.<br><br>Key features include:<br><br>Interactive diagnosis and treatment diagnosis<br>Full guideline text with bookmark and notes function<br>Sharing content with colleagues<br>Access to other useful resources<br>Disclaimer<br><br>The app is intended to assist healthcare professionals and allied healthcare professionals as an educational tool to provide information that may support them in providing care to patients. Patients or other community members using this app shall do so only after consultation with a health professional and shall not mistake this guideline as professional medical advice. The pathway must not substitute seeking professional medical and health advice from a health professional.<br>The pathway may not apply to all situations and shall be interpreted in the light of specific clinical situations and resource availability. It is up to every clinician to adapt this guideline to local regulations and to each patient’s individual circumstances and needs.
