彰化基督教醫院失智共同照護中心團隊偕同高雄科技大學建立國家衛生研究院失智症雲端護照，參考美國神經學學會建議之臨床失智症照護內容，做為雲端護照內容架構。
失智是一個持續性的照護過程，需要個案、家屬、多重專業團隊的共同討論與合作。本護照在個資無虞的情況下，上傳個案在醫療端及照護端的資訊至雲端平台，各方可即時檢視、共享決策。使照護連續不中斷，達到全人、全程醫療照護，守護每位失智個案和照顧者。
