## Description

**Optum** is a personalized health care management app that helps eligible members stay on top of their care. Find providers, schedule and manage appointments, access health information, review test results, handle prescriptions and connect with care teams from one secure place.

Depending on eligibility, users can also access Care Guides, nurses, wellness coaches and other support professionals through chat, messaging or phone. Optum helps members navigate benefits, coordinate care, understand health information and work toward individual wellness goals. This service is not intended for emergency or urgent care.
