Application for remote monitoring of Heart Failure patients.
This app is only for use by patients who have been diagnosed with Heart Failure (HF) within South Eastern Sydney Local Health District (SESLHD) and referred to the Remote Patient Monitoring – Heart Failure program.
 
This app is designed to help you track your blood pressure levels, heart rate, blood oxygen saturation rates and weight from the comfort of your home, and share the information with your care team.
