## Description

Catholic Health's MyChart-powered app lets Long Island patients manage their health from anywhere. Sign in with your Catholic Health MyChart credentials to find a doctor, schedule and manage appointments, check in virtually, request prescription refills, and securely communicate with your care team.

Review test results, notes from your care team, medications, immunization history, and other health information. You can view and pay bills, and upload health data (including data from Apple Health) when enrolled in self-tracking programs. If you don’t have a MyChart account yet, visit mychart.chsli.org to sign up or call 631-465-6100 for an activation code. Long Live Long Island.
