Of je nu zelf ondersteuning nodig hebt of hulp verleent aan anderen. OZO verbindt snel de juiste contacten, maakt communicatie inzichtelijk en versoepelt samenwerken. 

- Verbind het netwerk: Verbind iedereen die voor jou belangrijk is in één netwerk. Waarbij jij als ondersteuner de zorgontvanger weer in haar kracht zet.

- Stuur berichten: Verstuur berichten wanneer het jou uitkomt. Iedereen ontvangt op hetzelfde moment dezelfde informatie, zo zijn afspraken duidelijk en voorkom je misverstanden.

- Afspraken: Plan afspraken of MDO's met de zorgontvanger en nodig iedereen uit die de zorgontvanger kan ondersteunen vanuit haar expertise.

- Thuismetingen: Deel meetwaarden, zoals gewicht, bloeddruk, saturatie, ACQ score of CCQ score.

- Diabetes beleid: Verstuur dagcurves en pas gemakkelijk het insulinebeleid aan.

- Uitvoeringsverzoek: Vul het uitvoeringsverzoek, deel en onderteken het digitaal

- Wondplan: Maak een wondplan aan om de voortgang van (chronische) wonden bij te houden, door foto's te delen.
