## Description

Horizon NJ MyCare is a mobile app for Horizon NJ Health and Horizon NJ TotalCare (HMO D-SNP) members in New Jersey. It puts essential health-management features in your pocket: view covered benefits (medical, pharmacy, behavioral health, dental, vision and more); view, download or request a new member ID card; change your PCP; find doctors, dentists, or other providers; view your Care Manager; and discover community resources.

For Horizon NJ TotalCare members, the app also provides access to Horizon EXTRA Benefits Card features, telemedicine, and other services. If you have Horizon BCBSNJ coverage through an employer or directly, download the Horizon Blue app instead.
