## Description

Complete child developmental assessment forms online with the Greater Manchester Early Years App. Parents, carers and health visitors can complete and review forms for children aged 0 - 2.5 years old. The forms have been digitised to help ensure children are developing as expected and provided with additional support when needed. 

 

For parents and carers, it will:

Make it easy to access and complete forms online – you will no longer need to worry about losing or forgetting the paper forms
Give you online access to your child’s development records whenever you need them
Provide videos and other guidance to support your child’s development
Enable your health visitor to share documents with you through the app
 

The Early Years project is a partnership between the NHS in Greater Manchester, the Greater Manchester Combined Authority (GMCA) and Local Authorities across the city-region.

 

Account creation for parents and carers

You will be able to create your account on the app once a member of your health visiting team confirms your contact details and sends you an email inviting you to create your account.
