Charm mPHR is a Personal Health Record (PHR) app for securely managing health information for you and your family on mobile. It helps you track current and past medications, vitals, allergies, immunizations, diagnoses, procedures, insurance, and clinical documents, and also supports appointments and questionnaires.

Its Health Passport visually presents your data and can be shared instantly with caregivers or doctors. The app also enables video consultations, lab results and visit summaries viewing, secure messaging with practices, and quick sharing of health records and receipts—streamlining care coordination.
