## Description

Elation Passport for Patients lets you manage health information and communicate with your provider using your Elation Passport account. Review lab results, visit summaries, stay up-to-date on your care plan, and easily send and receive messages with your provider.

Key features include 24/7 access to your medical history, allergies, current medications, and other health data, plus secure sharing of your clinical information with your care team. On iOS, you can upload images with Camera, and receive push notifications so you’re always in the loop.
