## Description

Take the guess work out of following your prescribed care plan and tracking your results.  The Chanl Care App provides a direct connection between patient's with a heart or pulmonary condition and the care team at THEIR OWN hospital or clinic.  

This app integrates with Apple Health, and allows you to share activity and vitals with your care team, if you wish.

IMPORTANT:  This app can only be used by patients at participating hospitals.  Ask your care team if they provide access to the Chanl Care App.
