## Description

Texas Health MyChart is a mobile patient portal that gives you secure access to your health information and care team. From your phone, you can review test results, request medication refills, view immunization history, stay in touch with your care team, and schedule or manage appointments.

Additionally, you can upload health and fitness data (including Apple Health data when enrolled in self-tracking programs), access your family's records, receive health reminders, and complete health history questionnaires. New users can create an account at MyChart.TexasHealth.org, and help is available at MyChartHelp@TexasHealth.org.
