## Description

与糖医护App集合了三一照护糖尿病患者的院内外血糖相关数据，是医护人员院外血糖管理的工具。对患者院外的血糖、血压、饮食、运动等数据实时掌控，持续追踪患者的院外数据，实时进行咨询回答和干预指导。将医疗服务从院内延伸到院外，使患者遇到问题有人问，出了问题有人管。

核心功能：
[患者管理]:可查看管理的所以患者的院内外数据，包括院外数据：血糖血压、饮食/运动/步数/病例记录数据等；院内数据：检查化验、既往史、医瞩用药、复诊信息等。基于这些数据进行院外管理。
[消息]:通过消息回答患者的咨询，及主动干预指导。
[饮食点评]:针对患者上传的饮食记录进行点评。
[患教文章]:专业糖尿病相关知识库可发送给患者，作为医护人员辅助干预的工具。
[预约管理]:查看管理患者的预约情况。
