## Description

厦门i健康是由厦门市卫生和计划生育委员会创造性地以“三师共管”为理念，进一步推进“分级诊疗”战略落地，并充分结合“互联网+”思想，将慢性病患者从线下引导到线上,实现互联网化健康管理的一款移动互联网产品。该产品将缓解大医院拥挤现象,并且为病患提供更科学的全程健康关照管理,解决医患间的失联问题，缓解医患间的失信问题。同时，家庭医生基于该平台功能及互联网化管理模式，亦可以有效监控患者健康指标，对患者进行健康干预、预约和转诊等多种科学化管理，大幅度提升患者的病情监管的及时性。
