## Description

APPatient™ is an end-to-end patient engagement app designed for Modernizing Medicine patients and providers. This Chrome extension provides secure mobile access to telehealth, medical records, and real-time communication with your healthcare team. Premium Patient Connect offers 24-hour online check-in to expedite intake.

Patients can chat with their care team, request prescription refills, view labs and test results, and log in with existing Patient Portal credentials. Providers can access EMA data from their phone, stay connected with patients, and deliver care on the go. Log-in assistance through your physician’s office is available.
