ConsentForm puts your clients' consent forms on your tablet or phone: health questionnaire, on-screen signature, traceability, then a generated PDF emailed to your studio. Designed with the practitioner's hand in mind.

WHO IS IT FOR? Tattoo artists, piercers, and permanent makeup artists, including microblading. You select your services in your profile, and the form adapts: tattooing, piercing, or permanent makeup, each with its own specific information and traceability.

WHAT THE APP DOES
• A complete form: identity, contact information, date of birth, consents, image rights.

• The minor option, activated based on the date of birth: the parent or guardian signs the authorization on screen, and you photograph their ID.

• The client signs on screen; the practitioner's signature, entered once in their profile, is added to each form.

• The client also fills out a QR code on their own phone: a single-use QR code, or a permanent QR code to display at your workstation.

• Traceability in the client's file: inks or pigments (name, reference, batch) via your digital cabinet or as a photo; needle batch entered manually; for piercings, jewelry batch or sterilizer code, sterilization cycle of the forceps.

• Hygiene certificate saved in your profile, visual reminder before expiry.

• A dashboard with your current files, badges that indicate a minor client or a health concern, and your activity statistics.

• ConsentForm is also available in the browser at consentform.fr.

THE HEALTH QUESTIONNAIRE
Allergies, conditions or illnesses, medication, skin conditions, susceptibility to discomfort: the client answers themselves, at their own pace. Each question leaves a record on the client's file, even if unanswered. From your profile, you choose which questions to ask.

YOUR DOCUMENTS
Each completed file becomes a time-stamped PDF, complete with your logo and both signatures, emailed to your studio. With the premium version, your files can also be viewed in the app: search by client, filter by year, download PDFs, export to CSV, and access each client's file.

PREMIUM, OPTIONAL
Purchasing the app is all you need to get started. A subscription, available on a take-it-or-leave-it basis, adds: client archives and files, sending PDFs and your post-session instructions to clients upon request, scheduled appointment reminders, emails rewritten in your voice, feedback requests, email alerts before your hygiene certificate expires, a client loyalty card, a personalized information sheet (your terms and conditions, your questions, the completed project in PDF format), photo documentation of traceability (needles, jewelry, permanent makeup session, disposable tweezers), and a public mini-site in your name: practical information, contact details, testimonials, a gallery of 24 photos and flash designs available for booking, online booking, and website traffic statistics.

YOUR DATA
The website, API, and database are hosted by OVH. Photos taken during the final stages (inks and pigments for all; needles and jewelry with the premium option) are not stored on our servers: they appear only in the PDF sent by email upon completion.
