The Eclipse Health App connects to your GP surgery through the Eclipse clinical support service, enabling your feedback and preferences to be shared with your wider healthcare team.

You can use the Eclipse Health App to:
- Complete personalised health and wellbeing questionnaires.
- Give insight to your healthcare team so they know how you are feeling.
- Enables you to provide feedback and insight related to your health and wellbeing.

Once you have answered your personalised health and wellbeing questionnaires on the app, the feedback is used to optimise your care plan and personalise services for your needs.
