## Description

Sentiatend - an Android mobile/Tablet based application to Cluster coordinators (CCs) for re-assessment of the identification of the risks, provide follow up support with referral management and closure of the HRP cases identified in the field by MM. <br>It employs screening for pregnant woman, lactating mothers, child and adolescent <br><br>Checklists that screen are unique for each beneficiary -pregnant woman , lactating mother, child and adolescent.  Once the identification of the risks is diagnosed, workflow is governed by referral protocols and next plan of action.<br><br>The platform contains module for :-<br><br>•	Assessment of the risks<br>•	Initiate referral action<br>•	Follow up of the referral case <br>•	Case closure<br>•	Stock supply management - ASHA<br>•	User activity for the day<br>•	Data Reports<br><br>The app checks for the follow up action done by the MM and adds the name to the due list of clients that CC should visit. <br><br>CC will do the HV assigned to their respective area . Follow up is done for each of the high risks cases and re-assess the risks identified by the MMs. If risks are identified and confirmed, then <br>•	Alert is sent to MC (SMS) <br>•	Referral action is initiated as required. <br>•	The data is synced back to cloud and available on the MM and MC device
