## Description

Wellbridge Health offers an innovative, interactive chronic disease management program designed to improve quality of life, reduce avoidable emergency room visits and hospitalizations, improve self-management of chronic disease(s), and lower healthcare spend.  <br><br>This program can also be used for behavioral health -- anxiety and depression -- in combination with chronic disease management or stand-alone.<br><br>The BridgIT - Wellbridge Health app combines simple 7-day/week remote symptom monitoring through our customizable smartphone app and weekly face-to-face HIPAA-compliant video chat telehealth coaching. All of our members are partnered with a dedicated master’s trained health coach. <br><br>Features:<br>-       Personalized app questions regarding symptoms of members’ chronic disease(s) and/or behavioral health <br>-       Customized health and behavior symptom prompts<br>-       Takes under 2 minutes daily<br>-       Simple and easy-to-use design<br>-       Answers feed into dashboard reviewed daily by member’s designated health coach<br>-       Member is prompted to complete app questions if not completed by agreed-upon time<br>-       Algorithm determines if daily app answers are outside baseline and require further attention<br>-       Coaches review answers outside baseline and reach out to members when required<br>-       Health coach discusses daily and trend data with members during weekly video-chat<br>-       Health coach partners with members using specific professional training and tools to improve self-management of chronic disease(s) and reduce avoidable ER visits and hospitalizations.
