## Description

eCareMD is a chronic care management platform designed to keep patients connected with their care teams. The eCareMD Patient App helps patients stay engaged with their health programs, communicate with care managers, and improve adherence to programs such as CCM (Chronic Care Management) and RPM (Remote Patient Monitoring).<br><br>Key Features:<br><br>• Stay connected with your care manager<br>• Track and log your health vitals<br>• Complete health assessments and screenings<br>• Manage care plans and reminders<br>• View medication and program adherence<br><br>Note: RPM features require a compatible Bluetooth medical device, such as a blood pressure monitor, glucose meter, weight scale, or pulse oximeter, supplied by your care program. All other features, including care communication, assessments, reminders, and care plans, work without any external hardware.
