## Description

When a loved one enters hospice, everything falls on your family at once. Epilogue brings
your whole care space together, so nothing gets missed, and the time you have left is
spent on what matters.
YOU SHOULD NOT HAVE TO FIGURE THIS OUT ALONE
When a loved one enters hospice, families are handed an enormous responsibility with
almost no infrastructure to support it. Someone has to track medications, relay updates to
relatives, remember what the nurse said, and field calls at all hours, usually the same one
person, over and over.
Everyone in the care space is trying to stay informed and contribute, but there is no shared
center. Updates get repeated across texts, calls, and emails. Details fall through the gaps.
The goal is simple: less time managing information, more time being present.
EVERYTHING YOUR CARE SPACE NEEDS, IN ONE PLACE
Care log
Track medications, symptoms, and observations in real time, in plain language. Get
reminders so nothing is missed, and see the full history so everyone in the care space

always has the same picture.
Capture the moments
Record memories, conversations, and small moments alongside the care log. The story of
this time belongs to your family, and Epilogue helps you keep it.
See the full picture
One shared calendar showing family visits, hospice visits, other caregivers, equipment
deliveries, and appointments, with reminders so nothing overlaps and nobody has to ask
what is happening today.
BUILT FOR EVERYONE IN THE CARE SPACE
The patient or primary caregiver
Sometimes the same person, sometimes not. Whoever is at the center, Epilogue gives
them a place to log, organize, and communicate so the weight does not fall on one person
alone.
Friends, Family and other Caregivers
Family near and far, close friends, and non-medical caregivers. Epilogue keeps everyone
informed and able to contribute in ways that actually help.
Your hospice team
If you choose to connect them, your hospice nurse or care coordinator can see what the
family is observing between visits, so everyone is working from the same picture.
BUILT BY PEOPLE WHO HAVE LIVED THIS
Sensitivity to the situation and respect for the patient are designed into every part of
Epilogue, not bolted on.
PRIVACY AND SECURITY
Your family information is private. Epilogue is built to healthcare data standards. All
information is encrypted in transit and at rest. Only the people you have explicitly invited to
the care space can access its content. No one else, including Epilogue staff, can view your
loved one personal care information without your consent.
Your information is never sold, never shared with third parties, and never used for analysis.
You decide who is invited, what role they have, and what they can see. You can change
someone role or remove them at any time. Face ID and fingerprint login are supported on
compatible devices, so you do not have to remember passwords during one of life
hardest moments
