## Description

CareCorner is designed to support the day to day coordination of care for people with disabilities, people with chronic health conditions or elderly people.<br><br>CareCorner allows everyone to operate on the same page and stay up to date with the care recipient’s needs.<br><br>The agency or individual mainly responsible to coordinate Care for a person or the Care recipient themselves subscribes and sets up an account via our website (www.carecorner.net.au). They then invite contributors who can access the customised Care Platform via the App.<br><br>Agencies can sign up multiple clients at a time. <br><br>Nurses and Support workers can:<br>- review an up to date Nursing Care Plan.<br>- Add observations and interventions as the day goes on<br>- share progress notes with the whole team for a smooth shift handover<br>- review daily reports to ensure all care tasks are complete<br>- review custom reports to identify any trends<br>- easily access emergency contacts and plans<br>- sign off the administration of medications<br>- submit incident reports<br>- link the progress notes to goals<br><br>Agencies :<br>- save time due to more efficient reporting<br>- keep their coordinators up to date with live data access<br>- save money due to reduction in paper use<br>- save time and money by lessening the administrative workload<br>- go above and beyond by including the whole family in a holistic care model.<br><br>Family members are supported by:<br>- getting live updates on their loved one’s progress<br>- being empowered to educate through data<br>- creating daily to do lists for the care team
